- In-Home Medical Assessment & Care of Patient & Home Needs - evaluating nutrition, safety, mobility, psycho-social needs, etc
- Generating a Care Plan Document - that you will use as a Blueprint to guide your care
- Making Recommendations for any medical supplies, aides, equipment that would optimize the care of the Patient
- Care Coordination - we are happy to communicate directly with your doctor/specialist to ensure that your Patient gets the best care possible.
- Teaching and Training your staff (caregiver) or yourself on the best way to provide Care to your Patient - vital signs, CPR, record keeping, minimizing errors
- Resources - access to videos, documents, etc made just for our clients
- Coaching - Direct phone access to Registered Nurse or Doctor as necessary for one week.
- Preventing Complications - Tremendous Insight into the common pitfalls that occur. Skin care to prevent bed sores, for example.
- Identifying problems quickly - spotting small problems before they become gigantic headaches and dealing with them promptly.
- What to do in an Emergency (Emergency Planning) - making sure you have a care Action Plan for an emergency. What do you do if there's a possible heart attack, stroke, low blood sugar, etc?
- Nursing Care - if you don't already have nurses, we may provide thoroughly trained nurses. (dependent on your location)
